Dr Coory's practice is bound by the Privacy Act 1988 (Cth), the Australian Privacy Principles, and the Health Records Act and equivalent state legislation.
What we collect.
Personal and health information collected during your consultation, surgery and follow-up, including: name, contact details, date of birth, Medicare and health-fund details, clinical history, imaging and results, and correspondence with your referring doctors.
How we use it.
To provide your care, to communicate with your referring GP and other treating clinicians, to bill your consultation or surgery, to comply with our legal obligations (including notifiable infections, AOA Joint Replacement Registry data submission, and AHPRA mandatory notifications where applicable), and to manage our practice.
How we store it.
Information is held in encrypted electronic records on secure Australian-hosted systems. Paper records, where used, are kept in locked storage. Access is limited to authorised staff.
Disclosure.
Information may be disclosed to your referring doctor, other treating specialists, hospitals where you are admitted, your health fund or Medicare for billing, the AOA National Joint Replacement Registry (for joint replacement procedures), and where required by law.
Your rights.
You may request access to your information, request correction of inaccuracies, or make a complaint about our handling of your information. Please direct any such requests to reception in writing.
Marketing.
We do not use your information for marketing purposes.
